SUPPORT COORDINATION

The NDIS Support Coordination helps people with a disability to make the best use of their support fundings. Support Coordination connects people with a disability with NDIS Providers and builds confidence and skills to use and coordinate support. Divine Human Care Support Coordination is a capacity building support and will help you understand and use your NDIS Plan to pursue your goals.

 

Divine Human Care Support Coordinators delivers support coordination of two levels:

 

LEVEL 1

Support connection – This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.

LEVEL 2

Support coordination – coordination of supports: This support will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

WORKING WITH A SUPPORT COORDINATOR

When you have connected with Divine Human Care, you will meet your support coordinator to talk about services that will be delivered and how to implement your funded support. Then the next step will be to set up the service agreement with you and Divine and Divine Human Care
